چکیده
Background: A plethora of data supports HPV-based screening to be the preferred strategy for cervical cancer prevention. The shift a more sensitive first-line test brings need effective triage up discussion. Currently, most algorithms apply cytology as HPV-DNA positive women. This study compared performance 7-type HPV-mRNA cytology. Methods: From 1 January 2019 until 31 December 2021, samples from 58,029 women were examined at University Hospital North Norway. total 30.5% (17,684/58,029) fulfilled criteria primary screening. All triaged by and followed-up according national guidelines through 2022. Additionally, was applied. endpoint histologically confirmed high-grade lesion (CIN2+). Results: 5.6% (990/17,684) had test, 97.2% (962/990) with valid results. 55.5% (534/962) abnormal (ASC-US+), 35.1% (338/962) test. 13.9% (134/962) CIN2+. sensitivity (CIN2+) versus 76.1% (102/134) 73.1% (98/134), p = 0.67. specificity 47.8% (396/828) 71.0% (588/624), < 0.001. PPV 19.1% (102/534) 29.0% (98/338), 0.001, respectively. number colposcopies per CIN2+ detected 5.2 3.1. Conclusion: HPV mRNA significantly specific than in Using this biomarker threshold colposcopy may better balance benefits harms

5-type HPV mRNA versus 14-type HPV DNA test: test performance, over-diagnosis and overtreatment in triage of women with minor cervical lesions
BACKGROUND Repeat cytology and HPV testing is used in triage of women with minor cytological lesions. The objective of this study was to evaluate 14-type HPV DNA and 5-type HPV mRNA testing in delayed triage of women with ASC-US/LSIL. METHODS We compared a DNA test (Roche Cobas 4800) and an 5-type mRNA test (PreTect HPV-Proofer). In total 564 women were included in the study. RESULTS The se...

BACKGROUND In Norway, repeat cytology and HPV testing comprise delayed triage of women with minor cytological lesions. The objective of this study was to evaluate HPV DNA and HPV mRNA testing in triage of women with an ASC-US/LSIL diagnosis. MATERIALS AND METHODS We used repeat cytology, HPV DNA testing (Cobas 4800) and HPV mRNA testing (PreTect HPV-Proofer) to follow up 311 women aged 25-69 ...
متن کاملEfficacy of HPV DNA testing with cytology triage and/or repeat HPV DNA testing in primary cervical cancer screening.
BACKGROUND Primary cervical screening with both human papillomavirus (HPV) DNA testing and cytological examination of cervical cells with a Pap test (cytology) has been evaluated in randomized clinical trials. Because the vast majority of women with positive cytology are also HPV DNA positive, screening strategies that use HPV DNA testing as the primary screening test may be more effective. M...
متن کاملHPV DNA testing with cytology triage in cervical cancer screening: Influence of revealing HPV infection status.
BACKGROUND Knowledge of cervical human papillomavirus (HPV) status might influence a cytotechnician's assessment of cellular abnormalities. The authors compared original cytotechnicians' Papanicolaou (Pap) readings for which HPV status was concealed with Pap rereads for which HPV status was revealed separately for 3 screening populations. METHODS Previously collected cervical Pap smears and c...

High-risk human papillomavirus (hrHPV) testing has a higher sensitivity but lower specificity than cytology for detection of high-grade intraepithelial neoplasia (CIN). To avoid over-referral to colposcopy and overtreatment, hrHPV-positive women require triage testing and/or followup. A total of 25,658 women (30-60 years) enrolled in a population-based cohort study had an adequate baseline Pap ...
